Parents will know that when they enrol their child for daycare or school, they have to provide certain information about that child like allergies, likes and dislikes etc.
This particular daycare provided a questionnaire for the parents to fill out but the father in question presumed that these questions were all in related to his daughter, and so he answered as such.
In fairness, some of the questions are a little ridiculous which he was only too happy to point out.
